Using Jotforms – a form something like this could be added to the website. Every time a visitor fills out a form the office will get an email with the details and (optionally) a very plain format pdf attached to the email that can be saved electronically. The information also gets added to an excel spreadsheet so you don’t have to manually data enter the forms.
